The finding of a trepanning skull in Planadas, Tolima, a rare find in Colombian archaeology, motivates these reflections; a description of the finding and its context is made and similar practices in Colombia, the American continent, especially Peru, Bolivia, Mesoamerica, Europe, Asia and Oceania are studied and related. 
In Colombia since the beginning of preventive archeology in the 20th century, interdisciplinary research has been carried out in Colombia with restorers and conservatives, who have helped to safeguard the archaeological heritage. Actually there are numerous challenges for scientific research in archaeological contexts.
